PS5 No Man's Sky

Could someone help me out here? For many months now I've been discovering Systems and Planets and renaming those system and planets whenever possible to suit myself. I always make a point of discovering all the Fauna and at least 6 Waypoints on every planet I visit. However, recently when I returned to much of these planets, almost all of them had reverted back to their original names and all of the fauna and waypoints were showing 0 (Zero) discovered. It's as if I'd never been there. Can anyone shed any light as to why or how this could have happened?
